Root Canal vs Extraction: Should You Save or Remove the Tooth?

A root canal saves an infected natural tooth, while extraction removes it — saving the tooth is almost always preferable because it preserves chewing and stops neighbouring teeth from shifting, unless the tooth is too damaged to restore.

When a tooth is badly decayed or infected, the choice is usually between saving it with a root canal or removing it. Here is how to think about it.

Root canal vs extraction at a glance

Why saving the tooth is usually better

Keeping your natural tooth with a root canal treatment maintains normal chewing and prevents the neighbouring teeth from tilting into the gap. Modern root canals are virtually painless and, finished with a protective crown, can last for many years.

When is extraction the better option?

Removal makes sense when a tooth is cracked below the gum, has too little structure left to restore, or is severely affected by gum disease. If a tooth is removed, replacing it promptly with a dental implant or bridge protects your bite.

Frequently asked questions

Is it better to save a tooth or pull it?

Saving a natural tooth with a root canal is almost always preferable to removing it, as it preserves chewing and prevents neighbouring teeth from shifting. Extraction is reserved for teeth that cannot be restored.

Is a root canal or extraction more painful?

A modern root canal is virtually painless and relieves the pain of an infected tooth. An extraction is also done under anaesthesia, but leaves a gap that needs replacing.
